Summary
At Extension Healthcare we take your privacy and safeguarding Personally Identifiable Information (PII) seriously. Our Privacy Policy will let you know what PII we collect; how, why and when we collect it; what we use it for; who has access to it; why they have access to it; what we do to safeguard it and how you can access, change or delete your PII.

We don’t “sell” anything on the Extension Healthcare website. Our website is designed and dedicated to the sharing of Extension Healthcare related product/service offerings and educating interested parties and market participants about technology and how it can and is improving healthcare delivery and patient outcomes.

Outside of Cookies that help us monitor the effectiveness of our website and the interests of visitors all user information gathered has been given voluntarily.

Personally identifiable information (PII), as used in US privacy law and information security, is information that can be used on its own or with other information to identify, contact, or locate a single person, or to identify an individual in context.

What type of PII do we actively collect from people that visit our blog and/or website?
When volunteered we collect; first name, last name, company name, title, city, state, phone number and email address.

When and How do we collect PII?
We collect information from you when you register on our site to download informational “White Papers” or like materials, subscribe to a newsletter, respond to a survey, fill out a form or enter information on our site to ask a question or share a comment. This information is collected on Pardot and may be integrated into Salesforce.

How do we use your information?

  • To send periodic emails regarding our products and services as well as information about relevant changes in the healthcare services landscape
  • To allow us to better serve you in responding to your customer service requests
  • To administer a contest, promotion, survey or other site feature
  • Note: Although we do not foresee any reason for your PII to be distributed to anyone outside of our organization other than our service providers, in the event such distribution were to take place we would provide you with Just-in-Time Notice of the occurrence.

How do we protect your information?
Extension Healthcare’s goal is to protect your PII, submitted to us through our website(s). We maintain administrative, technical and physical safeguards to protect against unauthorized disclosure, use, alteration or destruction of the PII you provide on this or any other Extension Healthcare website. We also employ resonable technologies to help keep the PII you provide on this side secure.

Your PII is contained behind secured networks and is only accessible by a limited number of persons who have special access rights to such systems and are required to keep the information confidential.

Cookies
We use Cookies. Cookies are small files that a site or its service provider transfers to your computer’s hard drive through your Web browser (if you allow) that enables the site’s or service provider’s systems to recognize your browser and capture and remember certain information.

We use cookies to help us compile aggregate data about site traffic and site interaction so that we can offer better site experiences and tools in the future.  We also may use trusted third party services like Google Analytics that track this information on our behalf. We also use Pardot, the official B2B marketing application for Salesforce. Pardot uses cookies as it collects PII data. We use both Pardot and Salesforce to better streamline our sales process and serve our customers.

You can choose to have your computer warn you each time a cookie is being sent, or you can choose to turn off all cookies. You do this through your browser (like Internet Explorer) settings. Each browser is a little different, so look at your browser’s Help menu to learn the correct way to modify your cookies.

Third Party Disclosure
We will not sell, trade or otherwise transfer to outside parties your PII unless we provide you with advance or Just-in-Time notice. This does not include website hosting partners and other parties who assist us in operating our website, conducting our business, or servicing you, so long as those parties agree to keep this information confidential. We may also release your information when we believe release is appropriate to comply with the law, enforce our site policies, or protect ours or other’s rights, property, or safety.

We reserve the right to transfer any information we have about you in the event we sell or transfer all or a portion of our business or assets (including through bankruptcy).

We do not include or offer third party products or services on our website at this time. Our website contains links to other sites. While we try to link only to sites that share our high standards and respect for privacy, we are not responsible for the content, security, or privacy practices employed by other sites.

California Online Privacy Protection Act (CalOPPA)
CalOPPA is the first state law in the nation to require commercial websites and online services to post a privacy policy.   • We have added a link to our Privacy Policy on our website’s home page
  • Our Privacy Policy link includes the word ‘Privacy’, and can be easily found on the page specified above

How does our site handle do not track signals?
We honor do not track signals. We do not track, plant cookies, or use advertising when a Do Not Track (DNT) browser mechanism is in place.

Does our site allow third party behavioral tracking? 
Yes. We utilize Google Analytics as detailed under “Cookies”.

COPPA (Children Online Privacy Protection Act)
This website is not directed to children under the age of thirteen and we do not knowingly collect PII from children under the age of thirteen on this site or other Extension Healthcare sites. If we become aware that we have inadvertently received personal information from a visitor under the age of thirteen, we will delete the information from our records. If as a Parent or responsible Party to a child under the age of thirteen, you believe that the child has placed PII onto our site, please contact marketing@extensionhealthcare.com and we will delete this information within three (3) business days.

Fair Information Practices
The Fair Information Practices Principles form the backbone of privacy law in the United State and the concepts they include have played a significant role in the development of data protection laws around the globe. Understanding the Fair Information Practice Principles and how they should be implemented is critical to comply with the various privacy laws that protect personal information.

In order to be in line with Fair Information Practices we will take the following responsive action, should a data breach occur: 

  • We will notify the users via email within 7 business days
  • We will notify the users via in site notification within 7 business days

We also agree to the individual redress principle, which requires that individuals have a right to pursue legally enforceable rights against data collectors and processors who fail to adhere to the law. This principle proffers not only do individuals have enforceable rights against data users, but also that individuals have recourse to courts or a government agency to investigate and/or prosecute non-compliance by data processors.

CAN-SPAM Act
The CAN-SPAM Act  is a law that sets the rules for commercial email, establishes requirements for commercial messages, gives recipients the right to have emails stopped from being sent to them, and spells out tough penalties for violations.

We collect your email address in order to:

  • Send information, respond to inquiries, and/or other requests or questions
  • Market to our mailing list or continue to send emails to our clients after the original transaction has occurred

To be in accordance with CAN-SPAM we agree to the following:

  • Not to use false, or misleading subjects or email addresses
  • Identify the message as an advertisement in some reasonable way
  • Include the physical address of our business or site headquarters
  • Monitor third party email marketing services for compliance, if one is used
  • Honor opt-out/unsubscribe requests quickly

We do not disclose information to third parties. Period. Any user questions or comments submitted to us may become available in the public sections of the website, including a section entitled Frequently Answered Questions, immediately after such questions or comments are submitted to us. If we publicize any user questions, such questions will not contain facts that identify the user or the specific parties involved. We also reserve the right to use any user comments submitted to us for promotional purposes and by submitting such comments to us you agree that we may do so. We may release any user information if required to comply with law; to enforce or to apply the website Terms and Conditions; or under the good faith belief that disclosure is otherwise necessary or advisable including, without limitation, to protect the rights, properties, or safety of us or our users.

We give you a choice as to the type and amount of private information we have, as follows: Volunteered Information: You can always choose not to provide certain information to us. If you choose not to provide personal information, you can still use many parts of the website. However, you will not be able to take advantage of many of our features.
